Changes effective December 1, 2026

BlueCross BlueShield of South Carolina is expanding its collaboration with Evolent. These programs are intended to help providers effectively deliver quality patient care using clinical criteria based on nationally recognized guidelines to promote evidence-based practices.  

What does this mean for you?

Effective December 1, 2026, select interventional cardiovascular and physical medicine services will require prior authorization through Evolent. 

The Interventional Cardiovascular program will apply to all provider specialties for the following non-emergent services for BlueCross BlueShield of South Carolina and BlueChoice® HealthPlan of South Carolina Commercial/Exchange and Healthy Blue℠ adult members being rendered in provider offices, ambulatory surgical centers, outpatient hospital or inpatient hospital setting:

• Cardiac catheterization and intervention

• Electrophysiology

• Vascular radiology and intervention

• Cardiac surgery

• Vascular surgery

The Physical Medicine program will apply to all provider specialties for the following non-emergent services for Healthy Blue℠ members of all ages being rendered in provider offices, outpatient hospital settings and comprehensive rehabilitation facilities:

• Physical Therapy

• Occupational Therapy

• Speech Therapy

​Prior authorization requests for these services can be submitted to Evolent:

• Via the Evolent portal at evolent.com/provider-portal, select RadMD.
