Program changes effective February 1, 2026 |
Louisiana Healthcare Connections has partnered with Evolent to implement a new prior authorization program. This interventional cardiovascular program is intended to help providers effectively deliver quality patient care using clinical criteria based on nationally recognized guidelines to promote evidence-based practices. |
What does this mean for you? |
The Evolent interventional cardiovascular program will apply to all ordering provider specialties for the following non-emergent elective procedures for adult Louisiana Healthcare Connections Exchange members:
Beginning February 1, 2026, prior authorization requests for these services can be submitted to Evolent:
