Effective October 1, 2025, prior authorization for Tufts Health Plan Medicare Preferred (TMP) members will be required for the following non-emergent, MSK surgeries and IPM procedures when rendered in provider office, outpatient hospital, inpatient* or ambulatory surgery center settings (*MSK only):
Cervical decompression, fusion and artificial disc replacement Lumbar microdiscectomy, decompression, and fusion Sacroiliac joint fusion Hip, knee, and shoulder surgeries Cervical/thoracic, lumbar/sacral interlaminar epidurals Cervical/thoracic, lumbar/sacral transforaminal epidurals Cervical/thoracic, lumbar/sacral facet joint blocks Cervical/thoracic, lumbar/sacral radiofrequency (RF) neurolysis and cervical spine surgery Prior authorization requests for these services can be submitted to Evolent:
