Program changes effective November 1, 2025 |
Independent Health has partnered with Evolent (formerly NCH) to implement a new prior authorization program. This radiation oncology management program is intended to help providers effectively deliver quality patient care using clinical criteria based on nationally recognized guidelines to promote evidence-based practices.
Effective November 1, 2025, select radiation oncology therapy treatments will require prior authorization through Evolent prior to being rendered in an outpatient setting. |
What does this mean for you? |
Evolent will provide management services for the following radiation oncology therapy treatments for Independent Health Commercial and Medicare members aged 18 years of age and older for primary and metastatic cancers being treated with:
Low-dose-rate (LDR) Brachytherapy High-dose-rate (HDR) Brachytherapy Image Guided Radiation Therapy (IGRT) Intensity Modulated Radiation Therapy (IMRT) Proton and Neutron Beam Radiation Therapy Three-dimensional Conformal Radiation Therapy (3D-CRT) Two-dimensional Conventional Radiation Therapy (2D) Stereotactic Radiosurgery (SRS) Stereotactic Body Radiation Therapy (SBRT) Prior authorization requests for these services can be submitted to Evolent:
